Check-in Forms

For your first visit with us, please print out the Registration Packet below, and complete and fax to us in advance of your first visit, with your updated vaccination records for each pet.  Our fax # is 301-373-5155. If you are new to our Doggie Daycare program and wish to participate, please also complete the Group Playtime Profile.

On check-in day, please bring your pet on a leash or in a pet carrier and a copy of your current vaccination records if not already on file with us.  Please arrive at least 15 minutes prior to any closing times to allow time for pet check-in. All food, treats, and medications should be labeled with your last name, however, we do not guarantee return of any items (except unused medication), due to the huge volume of materials that typically arrive on any given day.  We try our best.
